Good question, as an MD I have only come across this one time. It was in Med school and the woman I was examining simply said, " I live a consensual alternative lifestyle. There has been no domestic abuse."
I have several colleagues who have encountered the same issue and have never heard anyone be judgmental or crude. Most healthcare providers are professionals and have the education and training to deal with patients who live outside "the normal confines of mainstream sexuality".
